Payment is due at the time of service. New patient appointments (90 minutes) are $250 and follow-up appointments are $150 (30-45 minutes). Fees are the same for in-person and phone/web appointments. Cash, check, and credit card/debit/HSA cards are all accepted forms of payment. Dr. Wade does not bill insurance directly but will gladly provide a superbill for patients to submit to their insurance (applicable to in office visits in Portland, OR only). Natural medicines are typically not covered by insurance plans directly, but HSA/FSA accounts can be utilized. It is the responsibility of the patient to understanding their insurance coverage.
Credit card information will be taken at the time of booking to hold the appointment. No charge will be made unless a cancellation within 48 hours of the appointment is made or the patient doesn’t show up for the appointment.
It’s very difficult to fill last minute cancellations. Kindly, give 48 hours notice if you wish to cancel your appointment. Cancellations made with less than 48 hours notice will incur a $125 cancellation fee for new patient appointments and a $75 cancellation fee for follow-up appointments.
Please contact your pharmacy at least one week prior to needing a refill on any prescription medication. Urgent refill requests will incur a $15 fee.
